Community Transformation Grants - To reduce death and disability from the five leading causes of death through the prevention and control these conditions and their risk factors. Recipients will select from a menu of interventions across the health and wellness spectrum, each of which can prevent or control chronic conditions. Each recipient will choose interventions that will help to achieve health equity, eliminate disparities, and improve the health of all groups. More specific goals within these broad goal areas will be provided in funding announcements


National Dissemenation and Support for Community Transformation Grants - TTo support the efforts of the CTG program by funding national non-governmental organizations with a network of community based organizations.

Who is eligible to apply/benefit from this assistance?
Applicant Eligibility
Applications may be submitted by State or local governments or their Bona Fide Agents (this includes the District of Columbia, the Commonwealth of Puerto Rico, the Virgin Islands, the Commonwealth of the Northern Marianna Islands, American Samoa, Guam, the Federated States of Micronesia, the Republic of the Marshall Islands, and the Republic of Palau). Eligible applicants also include public and private nonprofit organizations, for profit organizations, small, minority, women-owned businesses, universities, colleges, research institutions, hospitals, community-based organizations, faith-based organizations, Federally recognized Indian tribal governments, Indian tribes, and Indian tribal organizations. Additional guidance may be provided in individual program announcements.

Credentials/Documentation
Applicants must document the need for assistance, state the objectives of the project, outline the method of operation, describe the evaluation procedures and provide a budget justification for funds requested. Costs for nonprofit recipients will be determined in accordance with HHS Regulations, 45 CFR Part 74, Subpart Q. This program is excluded from coverage under OMB Circular No. A-87. This program is excluded from coverage under 2 CFR 200, Subpart E - Cost Principles.
What is the process for applying and being award this assistance?
Pre-Application Procedure
Executive Order 12372 may apply to the Community Transformation Grants, but does not applo to the National Dissemenation Support for Community Transformation Grants. Environmental impact information is not required for this program. This program is eligible for coverage under E.O. 12372, "Intergovernmental Review of Federal Programs." An applicant should consult the office or official designated as the single point of contact in his or her State for more information on the process the State requires to be followed in applying for assistance, if the State has selected the program for review.

2 CFR 200, Uniform Administrative Requirements, Cost Principles, and Audit Requirements for Federal Awards applies to this program.
Award Procedure
Applicable to both programs - An objective review panel will evaluate complete and responsive applications according to the evaluation criteria listed in funding opportunity announcement. Successful applicants will receive a Notice of Award (NOA) from the CDC Procurement and Grants Office. The NOA shall be the only binding, authorizing document between the recipient and CDC. The NOA will be signed by an authorized Grants Management Officer.

How may assistance be used?
Both programs - Funds may be used to support intensive approaches to chronic disease prevention and control described in Section “0.50” above. The use of and use restrictions are as follows: Recipients may only expend funds for reasonable policies, systems and environmental program purposes, including personnel, travel, supplies, and services, such as contractual to reduce risk factors, prevent and delay chronic disease. Recipients may not use funds for research. Recipients may not use funds for clinical care. Recipients may not generally use funding for the purchase of furniture or equipment. Any such proposed spending must be identified in the budget. The recipient must perform a substantial role in carrying out project objectives and not merely serve as a conduit for an award to another party or provider who is ineligible. What are the requirements after being awarded this opportunity?
Reporting
Applicable to both programs - Annual and semiannual progress reports are required. No cash reports are required. Applicable to both programs - Annual or semiannual progress reports are required. Applicable to both programs - Final financial status and performance reports are required 90 days after the end of a budget period. Programmatic performatnc monitoring is a requirement of these cooprative agreements.
Auditing
In accordance with the provisions of 2 CFR 200, Subpart F - Audit Requirements, non-Federal entities that expend financial assistance of $750,000 or more in Federal awards will have a single or a program-specific audit conducted for that year. Non-Federal entities that expend less than $750,000 a year in Federal awards are exempt from Federal audit requirements for that year, except as noted in 2 CFR 200.503.
Records
Financial records, supporting documents, statistical records, and all other records pertinent to the grant program must be kept readily available for review by personnel authorized to examine PHS grant accounts. Financial records, supporting documentation, statistical records, and all other records pertinent to an award shall be retained for a minimum of 3 years, or until completion and resolution of any audit in process or pending resolution. In all cases records must be retained until resolution of any audit questions. Property records must be retained in accordance with 45 CFR 92.42 requirements.
