PPHF - 2015 - Evidence-Based Falls Prevention Programs Financed Solely by 2015 Prevention and Public Health Funds (PPHF-2015)

 

This funding opportunity is designed to increase the number of older adults and adults with disabilities at risk for falls who participate in evidence-based community programs to reduce falls and falls risk, while concurrently increasing the sustainability of these programs through innovative funding arrangements. The awards will be made in the form of cooperative agreements because AoA will be substantially involved.

Who is eligible to apply/benefit from this assistance?
Applicant Eligibility
Eligible applicants are domestic public or private non-profit entities including state and local governments, Indian tribal governments and organizations (American Indian/Alaskan Native/Native American), faith-based organizations, community-based organizations, hospitals, and institutions of higher education. Current 2014 PPHF Fall Prevention grantees are not excluded from this funding opportunity but must provide a strong rationale for the need for additional funding. Individuals, foreign entities, and sole proprietorship organizations are not eligible to compete for, or receive, awards made under this announcement.
