Partner Support for Heart Disease and Stroke Prevention

 

The purpose of this program is to provide funds for organizations with national reach (the capacity to guide the Nation in taking action) to support implementation of national, state, and local level strategies that align with select recommendations from the Update to A Public Health Action Plan to Prevent Heart Disease and Stroke and Million Hearts™ initiative.
