National Project Management Of The Healthcare Communities Knowledge Gateway

 

The objectives of this collaborative agreement is to provide technical assistance to HRSA grantees and other providers of healthcare services by supporting the Healthcare Communities Knowledge Gateway, a robust knowledge and content management system portal, and related quality improvement efforts that bridge healthcare, public health, local and state based community organizations in improvement efforts across the United States. Healthcare Communities Knowledge Gateway provides support to the following: (a) Patient Safety and Clinical Pharmacy Services Collaborative (PSPC) participant organizations and related community based activities; (b) the Donation and Transplantation Community of Practice (DTCP) continued growth and success in saving lives and increasing awareness of the need for additional donors; and (c) information resources for the State Health Access program.

Who is eligible to apply/benefit from this assistance?
Applicant Eligibility
Public, private, for profit and not-for-profit entities are eligible to apply. Federally recognized Indian Tribal Government and Native American Organizations are eligible to apply. The applicant must have the ability to provide technical support and development; national project management of community based improvements, results sharing with automated analysis, and associated resources; and improve community resources including payer and health information technology systems.

Beneficiaries include current users such as eligible organizations and a variety of HRSA grantees including but not limited to: health centers, Counties and States, transplant hospitals and centers, qualified organ procurement organizations, educational institutions, state and local health departments, public health and other private entities.

This program is excluded from coverage under OMB Circular No. A-102. OMB Circular No. A-110 applies to this program. HRSA requires all applicants to apply electronically through Grants.gov.
Award Procedure
All qualified applications will be forwarded to an objective review committee. Based on the advice of the objective review committee, the HRSA program official with delegated authority is responsible for final selection and funding decisions. Notification is made in writing by a Notice of Award.

This cooperative agreement will provide funding for up to 3 years. Approximately $1,000,000 is expected to be available annually to fund one cooperative agreement. Applicants may request up to $1,000,000 per year. Funding beyond the first year is dependent on the availability of appropriated funds in subsequent fiscal years, awardee satisfactory performance, and a decision that continued funding is in the best interest of the Federal government.

How may assistance be used?
The cooperative agreement recipient operates and manages the Healthcare Communities Knowledge Gateway main portal and resources center, results-sharing site, and help desk or create an equivalent system without disruption of service to the current users such as eligible organizations and a variety of HRSA grantees including but not limited to: health centers, Counties and States, transplant hospitals and centers, qualified organ procurement organizations, educational institutions, state and local health departments, public health and other private entities. What are the requirements after being awarded this opportunity?
Reporting
The awardee must submit a progress report to HRSA on a quarterly basis. Sample text describing the report: This report has two parts. The first part demonstrates grantee progress on program-specific goals. The second part collects core performance measurement data including performance measurement data to measure the progress and impact of the project. Further information will be provided in the award notice. Cash reports are not applicable. The awardee will be required to submit performance and progress reports as well as status-federal financial reports (see the program announcement and notice of award for details for each required report). The awardee must submit a quarterly electronic Federal Financial Report (FFR) Cash Transaction Report via the Payment Management System within 30 days of the end of each calendar quarter. A Federal Financial Report (SF-425) according to the following schedule: http://www.hrsa.gov/grants/manage/technicalassistance/federalfinancialreport/ffrschedule.pdf. A final report is due within 90 days after the project period ends. If applicable, the awardee must submit a Tangible Personal Property Report (SF-428) and any related forms within 90 days after the project period ends. New awards (“Type 1”) issued under this funding opportunity announcement are subject to the reporting requirements of the Federal Funding Accountability and Transparency Act (FFATA) of 2006 (Pub. L. 109–282), as amended by section 6202 of Public Law 110–252, and implemented by 2 CFR Part 170. Grant and cooperative agreement recipients must report information for each first-tier subaward of $25,000 or more in federal funds and executive total compensation for the recipient’s and subrecipient’s five most highly compensated executives as outlined in Appendix A to 2 CFR Part 170 (FFATA details are available online at http://www.hrsa.gov/grants/ffata.html). Competing continuation awardees, etc. may be subject to this requirement and will be so notified in the Notice of Award. Expenditure reports are not applicable. Performance monitoring is not applicable.
Auditing
In accordance with the provisions of OMB Circular No. A-133 (Revised, June 27, 2003), "Audits of States, Local Governments, and Non-Profit Organizations," nonfederal entities that expend financial assistance of $500,000 or more in Federal awards will have a single or a program-specific audit conducted for that year. Nonfederal entities that expend less than $500,000 a year in Federal awards are exempt from Federal audit requirements for that year, except as noted in Circular No. A-133.
Records
Grantees are required to maintain grant accounting records 3 years after the date they submit the FFR. If any litigation, claim, negotiation, audit or other action involving the award has been started before the expiration of the 3-year period, the records shall be retained until completion of the action and resolution of all issues which arise from it, or until the end of the regular 3-year period, whichever is later.
