National Organizations for Chronic Disease Prevention

 

The purpose of this competitive supplement cooperative agreement is to enlist a national organization for chronic disease prevention to work in collaboration with CDC and the USAPI to improve the health of the people of the Pacific region by establishing the NCD HRSA-Model Collaborative, which is based on the Chronic Care Model that grounded the Health Resources and Services Administration (HRSA) Health Disparities Collaborative.
